Student Health Services
will be closed 
Monday August 7th through Wednesday August 23rd.
We will reopen for orientation Thursday, August 24.

If you are a new incoming student under the age of 26,
you must complete the Immunization History Form under the forms tab
on your patient portal AND upload your supporting documentation through
the portal.
BOTH must be done and it is not complete until
the form is filled in and submitted and the immunization record is uploaded.

has flu vaccine.  If you missed the clinics. you can schedule an appointment to receive one.  Please call us at 202-319-5744 or log onto the student health portal and
schedule it on line.

The cost is $25 and can be billed to your student account

Immunization Records

All students under the age of 26 are required by DC Law to submit immunization records.

All students under the age of 26 that are being admitted to the University for Fall 2017 are required to submit immunization records by.  Students that fail to submit records or their records are incomplete by the assigned due date, will have a $100 fee assessed to their student account. Incoming freshmen are required to have the records in by July 1, 2017. Transfer students, graduate students and International students are required to have records submitted by August 1, 2017.  If we have not received your records or they are not complete, it will be reflected in your check list on Cardinal Station. It is your responsibility to check your Cardinal Station account regularly. 

All immunization information MUST be entered through the student health portal. You need to enter your vaccination dates on to the immunization form on your patient portal AND upload your supporting documentation.  If you have any problems with this, please email .

If you are a current student and have noted that your student account has a hold placed on it for "health" reasons, your immunization record may be incomplete or missing. Please contact Student Health Services at  if you have any questions.  This hold will prevent you from being able to register for the Fall 2017 semester.

Medical Consent

If you will be under 18 years of age when you start the semester, please have your parents  sign and complete the medical consent form. This form is required in order to treat you at Student Health Services.
