Immunization Requirements

Please refer to the university policy on immunization for a complete description of immunization requirements http://policies.cua.edu/studentlife/studenthealthsvcs/immunization.cfm.  The Catholic University of America complies with the DC Immunization requirements.

Immunization records are due for all incoming freshmen for fall 2014 by July 1, 2014.  Any student that has not submitted records by that date, or their records are not complete by July 1, will have a $100 late fee assessed to their account.  All transfer, graduate and International students need to submit their records by August 1, 2014.  A late fee of $100 will be assessed to their accounts after the August 1 deadline if the records are not submitted or are not complete.

All information must be entered on the required immunization form.  We WILL NOT accept records that are attached to the form.  It is the responsibility of the student and not their health care provider to return the form by the due date.

Once we receive your records and they are complete and entered into Cardinal Station, it will no longer be listed on your "To Do" list on your Cardinal Station account.  If you have not submitted a complete Immunization Record within 10 days of the first day of class, a restrictive hold will be placed on your account.  This hold will prevent you from being able to register for the following semester.

The 2014-2015 Immunization Form is available  on line for download . Please complete and return as soon as possible to Student Health Services.

Meningitis Vaccine Requirement
The Catholic University of America, requires that all freshmen document receipt of the meningitis vaccine or complete and submit the meningitis waiver form.
