Immunization Requirements
Please refer to the university policy on immunization for a complete description of immunization requirements http://policies.cua.edu/studentlife/studenthealthsvcs/immunization.cfm. The Catholic University of America complies with the DC Immunization requirements.
Immunization records are due for all incoming students for the spring 2013 semester by January 14, 2013. If your records are not received or are received and are not complete by the add/drop date of January 25, 2013, a $100 late fee will be charged to your Student Account.
All information must be entered on the required immunization form. We WILL NOT accept records that are attached to the form. It is the responsibility of the student and not their health care provider to return the form by the due date.
A message will appear on your "To Do"check list on Cardinal Station if we have not received your immunization records or if we have received them and they are not complete. If you have not submitted a complete Immunization Record within 10 days of the first day of classes, a restrictive hold will be placed on your account. This hold will interfere with your ability to register for the following semester.

Meningitis Vaccine Requirement
The Catholic University of America, requires that all freshmen document receipt of the meningitis vaccine or complete and submit the meningitis waiver form.
